Online Survey for Students: A collaborative initiative by AIIHPH and MAC
Students are hereby informed that an Online Survey concerning the ‘Situation Assessment of the Student Household during Lock-down’ has been initiated by the Department Statistics, All India Institute of Hygiene and Public Health, Kolkata in collaboration with the Maulana Azad College, Kolkata. The Data collected from the Students will be purely confidential and will be used for academic purpose only.

Instructions:
1. Enter your gmail address to enter the form.
2. All questions are fairly understandable. Please be honest to fill the form.
3. Once you submit it cannot be editable.
4. Barring to very few, all are in the ‘multiple choice’, you have to click one. If you click *’other’* the cursor will blink into a new line, and you have to describe your reply. You cannot move to next question without answering the same with description
5. There are some check-box forms of questions where more than one can be selected. Similar approach for *’others’* is to be taken here.
6. For the last section ‘mental health ‘click the most appropriate from the list which describes you truly.
7. If inadvertently you have submitted an unfinished form, please send us mail to statisticsaiihph@gmail.com
নির্দেশাবলি:
১. তোমার জিমেল ব্যবহার করে ফর্মটিতে প্রবেশ করতে হবে।
২. প্রতিটি প্রশ্ন খুবই সাধারণ, তাই সৎ ভাবে নিজের উত্তর দাও।
৩. একবার SUBMIT করলে আর পাল্টানো যাবে না।
৪ . বেশিরভাগই ‘multiple choice’ প্রশ্ন, একটিমাত্র উত্তরে ক্লিক করো। *’অন্য’* ক্লিক করলে বিশদে বলতে হবে; নয়ত পরের প্রশ্নে যাওয়া যাবে না।
৫.. কিছু কিছু ‘ check box’ প্রশ্ন, সেগুলোতে একটার বেশি উত্তরে ক্লিক করতে পারবে। *অন্য* ক্লিক করলে সেটা আগের মতই বিশদে লিখতে হবে
৬. মানসিক স্বাস্হ্য বিষয়ক প্রশ্নগুলোতে তোমার জন্য যেটি সবচেয়ে বেশি যথাযথ সেই উত্তরে ক্লিক করতে হবে।
